Solve : Unable to unlock workstation?

I have a workstation that when locked, never DISPLAYS the Windows log-in. It's only showing the background. I've replaced the user's mygina.dll and verified it was being used in the registry. We are not USING smart keys.

Any IDEAS?What diagnostics did you run?
Safe mode?
CRETE a NEW user account with same privilege level. Se if he same issue is found in the new account.
