Solve : Outlook Forms Checkbox?

I have CREATED a form with some CHECKBOXES.  My problem is that when the form is sent to a mail recipient, the checks are not retained.

Any ASSISTANCE would be appreciated.

/meNever mind.  I found my problem.  I had INSERTED a combo box not a checkbox.
