As for the original question, improving the quality of the video, I've never used Realplayer's downloader but when you view the video on Youtube, does it look OK? If so, and it's just the downloaded video that doesn't look right, it must be the software - either an option, or just bad software. Check the options and see if there's anything in there that may help, if not then I WOULD suggest trying out a different program to do this as the Realplayer software might just not be that great. I don't have a TV, don't get a newspaper, and don't read much news online, so I'm VAGUELY aware of the name Miley Cyrus, though not who she may be or what she may do, apart from "twerking".
